NHS Health Check Secondary Use Data Set

NHS Health Checks could prevent over 4,000 people a year from developing type 2 diabetes and detect at least 20,000 cases of diabetes or kidney disease earlier, allowing individuals to be better managed and improve their quality of life.

NHS Health Check Data Set

The Health and Social Care Information Centre (HSCIC) is working together with the Department of Health and NHS Diabetes and Kidney Care, to support the NHS in delivering the NHS Health Check programme. The HSCIC was commissioned to develop and support the implementation of the national secondary uses data standard which sets out to measure the delivery of key objectives of the NHS Health Check Programme.

The NHS Health Check data set was fully mandated by the Information Standards Board in June 2011. The Information Standards Notice was issued on 12 July 2011. The Implementation Support Pack includes the following:

NHS Health Check service requirements

General Practices and alternative service providers (including pharmacies and community outreach clinics) commissioned to conduct the NHS Health Check are required to routinely record clinical and administrative data in an electronic form.

IT system suppliers are required to extract patient-level information form from clinical records in line with the mandated data set and provide a quarterly submission to the HSCIC in line with the technical guidelines.

Commissioned service providers must electronically record NHS Health Check information by April 2012 and IT system suppliers must make their first quarterly submission by June 2012 and continue doing so every three months thereafter.

Background to NHS Health Check

The NHS Health Check programme aims to help prevent heart disease, stroke, diabetes and kidney disease. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions, should be invited (once every five years) to have a check to assess their risk of heart disease, stroke, kidney disease and diabetes and should be given support and advice to help them reduce or manage that risk.
