SPAD investigation reports

Following the Ladbroke Grove accident, the Health and Safety Executive (HSE) produced publicly available monthly signals passed at danger (SPAD) reports, from October 1999 until September 2005. After that, Transport Ministers agreed that HSE should produce quarterly SPAD reports and we have subsequently taken over responsibility for this.

Following a review of the reports and consultation with the DfT we now provide a summary of the SPAD report quarterly in the National Rail Trends publication rather than in a separate report.

The last SPAD report published on this section was Q3 2009. The Q4 2009 SPAD statistics was reported in the National Rail Trends publication released on 11 February 2010.

The Rail Safety and Standards Board (RSSB) still publishes monthly, publicly available, SPAD reports.
