Coordinate My Care (CMC) is a clinical service underpinned by an electronic solution that coordinates care, giving patients choice and improved quality of life. This event is aimed at all end of life care staff, commissioners, GPs, social services, hospice staff, nursing home staff and voluntary organisations.
The event will be held on Thursday 26 July 2012 at the Education and Conference Centre at the Royal Marsden, Stewart’s Grove, SW3 6JJ. Cost is £100 per delegate, with group discounts available.
