Shared care protocol for patients on acetylcholinesterase inhibitors

NHS North Yorkshire and York have developed a shared care protocol between primary and secondary care so that if the patients on acetylcholinesterase inhibitors (AChEI) are stable they can be transferred back to the GP who will provide a yearly review of their AChEI as part of their routine dementia Quality and Outcomes Framework (QOF) review. In exchange patients with dementia under the Community Mental Health Team will have their QOF review incorporated into care coordination. The only additional paperwork is adding a heading ‘physical review’ to the care plan.
