Focus on: Preparing for End Stage Renal Disease
This document reports the findings of a project conducted by the Delivering Quality and Value team over the past year. The aim of the project is to answer the question, 'What are the aspects of an efficient clinical pathway from diagnosis to end stafe renal failure to assessment ad addition to the transplant list?' by focusing on one aspect of renal services; the year leading up to renal replacement therapy.

The document represents the first project output. It includes the approach, the idealised pathway, f u ndamental principles for delivering the pre-dialysis year idealised pathway, key characteristics of high quality, pre-dialysis care, measures for improvement, case studies and other important information. It will be followed by practical tools that have been co-produced with the NHS to help the wider service implement improvements for patients and staff.
