  • National Insurance number required

You can’t use this service if any of the following apply:

  • your original voucher has expired
  • your original voucher was lost or stolen more than a year ago
  • your child’s name or address has changed
  • you are claiming a European family benefit

If your name isn’t on a Child Benefit award notice

If you have parental responsibility for a child but your name isn’t on their Child Benefit award notice, you’ll need to use the ‘parental responsibility’ form to get a replacement voucher.

The replacement voucher will be sent to the person who gets Child Benefit for the child.
