#NOF team
Project Description:

The Neck of Femur (#NOF) team has developed a more effective management of elderly patient care.

This project sees the patient jointly cared for from admission by both the orthopaedic surgeons and geriatricians. A retrospective study is performed identifying the difference in outcomes between the #NOF approach and traditional methods. The main outcomes monitored were delays to surgery, mortality, length of stay and discharge destination. #NOF patients treated on a trauma ward with daily medical input faired better than those on a Geriatric ward with orthopaedic input.

'Am I OK?' website
Project Description:

The ‘Am-I-Ok?’ website addresses many questions young people may have about their own mental health.

The website helps young people recognize ‘at risk’ mental states, acting as a signpost to appropriate services. ‘Am-I-Ok?’ covers psychosis and schizophrenia in a suitable and easily identifiable manner. It also emphasises the symptoms and vulnerabilities young people with mental health problems manage. The site links directly to an NHS search engine, provides a list of useful websites and a one-way feedback system.

'An hour to save a life' project team
Project Description:

This project aims to significantly reduce healthcare associated infections at Musgrove Park Hospital through educational presentations.

It creates a communications experience to reduce infection rates enabling people to feel integral to the process. Their goals are to:

•    Fully engage clinical staff in the development and presenting team
•    Increase levels of isolation of patients when symptoms start
•    Reduce usage of the broad spectrum antibiotic ciprofloxacin

They adopt the best teaching techniques to ensure their message has a lasting effect.

'At a Glance' Fluid Balance Bar Chart
Project Description:

This new chart shows cumulative fluid balance totals in visually compelling bar format.

The detailed numerical data on standard cumulative charts are not easy to grasp quickly. This additional chart is designed for display on top of the usual charts. It provides ‘at a glance’ information, alerting staff to possible over or under-hydration. The extra chart takes seconds to complete after calculating 24 hr totals.

'Enjoying Life'
Project Description:

This group aims to maximise mental health wellbeing and encourage active involvement in accessing these services.

They provide a safe and supportive environment where the service user is exposed to available local area activities. Activities include healthy eating, exercise and social interests. They also invite outside agencies such as MIND, health trainers or Tai Chi instructors to present to the group. The activities help promote a healthy, holistic lifestyle.

'Fit 4 Life' project
Project Description:

‘Fit 4 Life’ is a health improvement project designed to address health inequalities in South Birmingham.

The ‘Fit 4 Life’ team is employed by the council and consists of three qualified and experienced exercise professionals. The project has been developed in the Northfield constituency as part of the Healthier Communities theme Group. This group is a pilot partnership for the city’s Local Area Agreement (LAA). The city’s LAA identified a widening gap in male life expectancy in the most deprived areas.

'Food for thought' project
Project Description:
The ‘food for thought’ project provides continuing and end of life care in a hospital setting.

Beech Ward is a thirty-bed ward within Doncaster Primary Care Trust. Nutrition in hospital has suffered from some adverse reports in the media. This has resulted in the Age Concern ‘hungry to be heard’ document.  Following this, the team participated in the directorates Essence of Care programme. They also undertook the Nutrition Benchmark, reviewing their current practice relating to the Nutritional intake of their client.

'Forget Me Not' Centre
Project Description:
This project involves an innovative service for younger people with early onset dementia.

They aim to create a culture of hope, recovery and an expectation of quality of life. This is achieved by ensuring that clients’ autonomy, self-determination and respect is central to every activity and interaction. ‘Occupation for health’ is their underpinning philosophy. ‘Occupation’ is how they spend their time and what they do in life. This has a direct impact on health, quality of life and happiness.

'Hard Day's Night'
Project Description:
This project is a novel ward simulation teaching session involving final year medical and nursing students.

‘Hard Day’s Night’ is easy to run within any teaching centre devoid of need for high-tech equipment. It aims to prepare students for the challenges of working as a team in a busy clinical environment. It highlights issues such as prioritisation, documentation and inter-professional communication. This session involves the creation of a mock ward that contains different working ‘scenarios’.

'Healthclick'
Project Description:
This project provides an improved information service for patients suffering with long-term conditions (LTC).

The team has designed an information prescription (IP) allowing a LTC patient to take better care of themselves. The list of LTC focused on includes:

•    Diabetes
•    Coronary obstructive pulmonary disorder
•    Coronary heart disease
•    Hypertension
•    Depression
•    Stroke
•    Lymphoedema
