Trigger Tool iconTrigger Tools are a means of conducting rapid structured casenote review to measure the rate of harm in healthcare.  Because they are a metric, they can also be used to track improvements in safety over time.

     Using the on-line Trigger Tool Portal

    The secure Trigger Tool Portal allows you to enter your case note reviews data and obtain automatic, live results analysis. You can then review the rate of harm for your organisation, and track improvements over time.

    Please note before registering to use the Trigger Tool Portal, you will need to be logged into the NHS Institute website.  If you don't have a profile, just click 'Log In' at the top of the screen and follow the instructions.

    Organisations outside NHS England

    The Trigger Tools Portal will be open for registrations from health organisations outside England from Spring 2010.
