Staff across the NHS work very hard to care for their patients, yet despite their efforts and good intentions, many patients suffer complications whilst under their care. The same is true in healthcare systems the world over. 

Increasingly, staff and patients are demanding action. They are proving that such events are often avoidable, and not just 'recognised complications'. Often it is about improving our systems and processes, making it easier for staff to do it right and minimising distractions from care delivery.
