H1. Chronic Conditions Guide and Carers

Liverpool Community Health recognises that employees who have long-term chronic conditions very often require additional support to enable them to stay in work. The Trusts has in place a number of policies that gives guidance to Line Managers and employees with regard to practical and compassionate support. These policies include Managing Sickness and Attendance policy which promote regular contact with the employee and early referral to Occupational Health services for support and advice, other supportive initiatives include phased returns to work to help the employee to return to work earlier. The Trust has a Reasonable Adjustments Policy which enables discussions around reasonable adjustments that can support early return to work or maintain someone in work whilst managing their health condition. The Flexible Working Policy allows the employee to request changes to their working patterns either on a temporary or permanent basis to enable them to manage their health condition and achieve a work-life balance more effectively. The Trust also has in place the provision of Occupational Health Services, In-house Physiotherapy service, access to internal mediation services and access to Staff Support/Counselling services.
