Call for applications: Early Implementer sites for the QIPP LTC Year of Care Funding Model

QIPP LTC Workstream has been working with the NHS, Social Care and Voluntary Sector to develop a Year of Care Funding Model aiming to support integration of care for people with LTC by aligning the funding flows and incentives with peoples’ need.  The team are now seeking Early Implementers to implement the model as “business as usual”, with central support and funding.

The application form and supporting documents are available:  The closing date for applications is Friday 25th May, 2012.  A WebEx will be held on Thursday 3rd May, 12.30 – 1.30pm to explain the LTC Year of Care funding model in more detail, make clear the requirements of the Early Implementer sites and the criteria for selection and to answer questions people may have.  Further information on how to participate in this is included in the application notes.
