Acts as lead professional with support
  • Applies integrated working skills as a lead professional within the context of integrated working
Takes part in/leads common induction
  • Disseminates knowledge from own experience, and receives knowledge and support for skills development from others within the integrated workforce
Takes part in/leads communities of practice
  • Actively contributes to developing and leading shared practice in the context of integrated working
Uses online directories
  • Identifies own knowledge requirements and seeks additional support through the use of online and other directories
Works with/acts as ‘episode co-ordinator’
  • Demonstrates integrated working practice with other professionals through interventions
