Welcome to the Condition Management Programme

If you want to return to work but feel that your health condition is getting in the way then the Condition Management Programme may be able to help.   Eastern Valleys Condition Management Programme covers the boroughs of Merthyr Tydfil, Caerphilly and Blaenau Gwent.
 
The programme is open to anyone claiming Incapacity Benefit or Employment Support Allowance, living with a mild to moderate health condition.  The referral must come from your personal advisor at the Jobcentre Plus.
 
The CMP team will work with you to help you explore what you can do to better manage your condition.  This site will give you an overview of the types of sessions we offer, introduce you to some of the team and fill you in on what you can expect if you take part in the Condition Management Programme.

A public consultation is underway until 28th January 2010. The Condition Management Programme forms part of the 'Pathways' initiative and anyone with experience of CMP or other 'Pathways' support options can give their views.

The programme has recently appointed Julie Smith  to the newly created role of Customer Contact Development Officer. As a former CMP customer herself, Julie’s input to the team is invaluable, from providing a customer perspective in decision making to meeting potential customers and informing them of the benefits of CMP.
