Equality and Diversity questionnaire Equality and diversity questionnaire

We have created a questionnaire to help us to produce a profile of our customers. We hope that this will make sure we provide an accessible service for all. It contains nine questions and we estimate that it should take no longer than five minutes to complete.
All information is confidential. The information you provide is anonymous and cannot be linked to you personally.

You can complete the equality and diversity questionnaire online or download a print version. For more information, read our Equality and diversity page.

Guidance index replaces document library Guidance index replaces document library

The changes are a direct result of user research that showed, among other things, that users want to view a full list of our guidance for organisations in one place on the website but that the document library was a confusing tool with too many irrelevant documents and unclear headings.
