The application form, fee and supporting documents for a passport name change

The Identity and Passport Service (IPS) has one standard form for all types of individual applications. The form comes with full instructions, whether you fill in the paper version or complete it online. Learn how to get the form and what fee and supporting documents you need to send IPS.

The cost of changing the name on your passport

For an adult passport it costs £77.50 for standard service, £112.50 for the one-week Fast Track service and £129.50 for the Premium one-day service. For a child passport it costs £49 for standard service, £96.50 for the one-week Fast Track service and £109.50 for the Premium one-day service.

The application form

How to get it

You can get the form in four ways. You can:

  • complete the application online, in which case IPS will print out the completed form and send it to you to sign and return with supporting documents
  • collect it from any Post Office branch that offers the Check & Send service
  • request an application form online and IPS will send it to you in the post - allow up to five working days for delivery
  • call the IPS Passport Adviceline on 0300 222 0000 and ask IPS to send it to you in the post - allow up to five working days for delivery

Signing the form

Only you need to sign the form if you are applying just to change your name. No one else needs to sign it, unless your appearance has changed considerably since your last passport, or your passport is lost, stolen or damaged.

Which name to sign

You should sign the form at section 9 with your new name. Use your new name even if you are waiting to get married or enter a civil partnership and have not yet officially changed your name.

The supporting documents

IPS needs to see evidence of all your name changes. You must send original documents, not copies. IPS will accept laminated documents as proof of name changes.

You will need to send:

  • completed application form
  • your current or last passport
  • two recent, identical photographs

and supporting documents as detailed below.

If you are waiting to get married or form a civil partnership

IPS needs form PD2, 'Passports for Newly Weds and Civil Partners'. For information about this form and how to get it, follow the link below.

If you have already changed your name

IPS will need to see documents showing the date of and reason for the name change.

If you have changed your name more than once IPS will need to see evidence of all changes. The documents must show a clear link between each change of name.

Marriage and civil partnership name changes

For married people, IPS will need to see marriage certificates.

For civil partners, IPS will need to see civil partnership certificates.

Going back to your maiden or unmarried name

If you have gone back to your maiden or unmarried name, IPS will need to see:

  • your birth certificate
  • a signed statement from you saying that you have gone back to your maiden name for all purposes
  • proof of the history of names (a decree absolute showing both names, a marriage certificate showing both names or a formal name change document)

Name changes connected with gender re-assignment

If you want to change the name in your passport because you have changed gender, IPS needs to see your change of name deed and one of the following:

  • a letter from your doctor stating that they expect your change of gender to be permanent
  • a gender recognition certificate
  • a new birth certificate

Name changes by deed poll

In all other cases IPS needs to see your change of name deed.
