Application forms for seafarer certificates
The application forms contain instructions for the certificates and documents needed. Please ensure you read these instructions fully before submitting an application. Failure to complete your application will cause a delay in processing.

THE CORRECT FEE – please click here to refer to our current fees list for the relevant fee for your application. If paying by cheque or postal order please make it payable to ‘Maritime and Coastguard Agency’ only. We accept debit and credit cards.
PHOTOGRAPHS - Please do not submit photos with a dark background as we are unable to issue your Certificate of Competency/Equivalent Competency. For operational reasons photographs with white backgrounds must be used.
SIGNATURE - please sign your usual signature. Ensure that the whole of your signature is contained within the outline of the box in the application form – this will be transferred to your new Certificate of Competency/ Certificate of Equivalent Competency.
SERVICE STANDARDS –
APPLICATION TRACKING - If you have downloaded your application form from this website or have the latest edition of the printed form, the form will have a unique number which can be found at the right hand side of the form. You can use this to check the progress of your application with our online checking system. If you submit an application form that has been copied you will not have a unique number and you will not be able to track your application online. Please note, this system does not apply to GMDSS, tanker endorsement, replacement or fishing applications.

COURIER – For new CoC applications only your documents will be returned by courier and this charge is included in the fee. It is important that you provide us with an address where you will be available to sign for your documents. Please also provide us with a telephone number so that the courier company can get in touch with you should you not be available. Failure to do so will result in a delay of you receiving your documents.
