Safety Alerts previously issued through the Safety Alert Broadcast System (SABS), will now be issued through the Central Alerting System (CAS). CAS replaces both the Safety Alert Broadcast System (SABS) and the Chief Medical Officer’s Public Health Link (PHL).
An on-line reporting facility introduced in 2004 enables NHS organisations to file defects and failures reports on-line. Alternatively the form is available for download here.
