Motoring

If you are applying for or currently hold a large goods vehicle or passenger carrying vehicle (group two) driving licence

Download the appropriate information leaflet or medical questionnaire to tell the Driver and Vehicle Licensing Agency (DVLA) about your medical condition or disability. The questionnaires include a consent form that must be signed, allowing DVLA to make medical enquiries into your fitness to drive.

Neurological conditions

For the following condition:

  • epilepsy

For the following conditions:

  • fits or blackouts

For the following condition:

  • severe and recurrent disabling giddiness

For the following condition:

  • parkinson's disease

For the following conditions:

  • narcolepsy or cataplexy

For the following conditions:

  • any chronic neurological condition (eg multiple sclerosis, motor neurone disease)
  • major or minor strokes
  • brain surgery, brain tumour, severe head injury
  • memory problems

For the following conditions:

  • chronic neurological conditions (eg multiple sclerosis, motor neurone disease)

Psychiatric conditions

For the following conditions:

  • any mental ill-health condition (including depression)
  • any psychiatric illness requiring hospital admission

For the following conditions:

  • alzheimer’s, cognitive impairment, dementia, memory problems

Alcohol and drugs

For the following condition:

  • dependence on or misuse of alcohol in the past three years

For the following condition:

  • dependence on or misuse of drugs in the past three years

Vision problems

For the following conditions:

  • sight in one eye only
  • visual problem affecting either eye

Diabetes

For the following conditions:

  • diabetes controlled by diet
  • diabetes controlled by tablets
  • diabetes controlled by insulin

Heart conditions

For the following conditions:

  • any heart condition other than innocent heart murmurs
  • peripheral arterial disease causing symptoms eg intermittent claudication
  • abdominal aortic aneurysm

Sleep disorders

For the following conditions:

  • sleep apnoea syndrome
  • narcolepsy or cataplexy   
  • any other condition which causes excessive daytime or awaketime sleepiness

Spinal injuries

For the following conditions:

  • severe spinal injuries 
  • paraplegia
  • tetraplegia

Cancers, tumours and other medical conditions

For the following conditions:

  • you must notify DVLA if you have had treatment for the following types of cancer or tumours in the past five years: Lung, melanoma, non-hodgkins lymphoma
  • AIDS
  • any other medical condition likely to affect ability to safely control a vehicle, eg amputation, impairment secondary to medication, chronic debilitation illness
  • behavioural problems eg asperger's syndrome

For the following condition:

  • chronic renal failure

For the following condition:

  • deafness (profound)
