There are two training packages available here. Both are intended to support staff in managing hospital discharge effectively.
This package supports good practice guidance: Discharge from Hospital: pathway, process and practice, published by the Department of Health in January 2003. The material currently consists of eight modules, which are based on the best principles of established good practice in hospital discharge. An additional module is being developed for use in emergency units.
In April 2003 the Department of Health introduced legislation - the Community Care (Delayed Discharges, etc.) Act 2003 - which builds upon these principles, providing new incentives for local partners to improve performance.
The Act places new duties on both NHS and social care partners around hospital discharge communications and planning, and introduces a daily reimbursement charge, payable by local authorities to the NHS, for any delays for which councils are solely responsible.
