Consent forms

  • Last modified date:
    19 October 2009

The Department is undertaking a review to identify and evaluate the impact on practice of existing DH guidance and forms (see the homepage for this section).  Whilst this is underway, the Department will not be updating existing forms or issuing new forms.

In the interim, Trusts are free to develop their own documentation, including their consent forms (using the DH model form if they so wish), to reflect the current legal position and reflect local practice as appropriate. The DH weblink below includes some advice on how legal terminology has evolved since the forms were produced, to help Trusts review their documentation to ensure they continue to have in place effective and legal consent processes

Download the following from the links below:

  • About the consent form
  • Consent form 1 - Patient agreement to investigation or treatment
  • Consent form 2 - Parental agreement to investigation or treatment for a child or young person
  • Consent form 3 - Patient/parental agreement to investigation or treatment (procedures where consciousness not impaired)
  • Consent form 4- Form for adults who are unable to consent to investigation or treatment
