Since the second half of the 20th century far more people are living into their seventies, eighties and beyond. But for many people increased longevity also means living with chronic conditions such as heart disease, stroke, cancer, arthritis, diabetes, and mental illness from middle to old age. The Expert Patients Programme is a lay-led self-management programme that has been specifically developed for people living with long-term conditions. The aim of the programme is to support people to increase their confidence, improve their quality of life and better manage their condition.
