AFPS Form 4 - In-Scheme AVC Application (DOC): The application form to apply for DIS, spouse's or all benefits AVC. Once completed and processed by SPVA, the decision to commence is FINAL and IRREVOCABLE.
AFPS Form 5 - Added Years Application (DOC): The application form to apply for Added Years. Once completed and processed by SPVA the decision to commence is FINAL and IRREVOCABLE.
Find out about Preserved Pensions: Veterans who have served in the Armed Forces since 1975 and did not qualify for an immediate pension may be entitled to a Preserved Pension (PP). It is incumbent upon prospective pensioners to approach Service Personnel Veterans Agency (SPVA) to identify whether they are entitled to a PP.
AFPS Form 8 - Claim Preserved Pension (DOC): Preserved pensions need to be claimed. This form should be completed 3 months before your 60th birthday and sent to SPVA.
