Defence Flight Safety Occurrence Report form & ASIMS User Guide

The Defence Flight Safety Occurrence Report should be used to report a Flight Safety related 'event'.  The Airprox Report, the Birdstrike Report, and the AIR(C) Report are complementary forms for reporting specific flight safety events.

The Aviation Safety Information Management System, ASIMS, is the database of all flight safety occurrences.  The ASIMS User Guide provides guidance to all those personnel who are a link in the 'flight safety chain'.  The User Guide also contains guidance on completing a D-FSOR (the D-FSOR Aide Memoire).
