THE POLICE NATIONAL DRINK/DRUGS
DRIVE PROFORMA
This is the first time that police in England and Wales have used one
identical form in all forces for dealing with drink and drugs driving
offences. While it is being called the "national" form, it covers
only forces in England and Wales. It is divided into sections A to E.
The designation MG DD shows that it is included in the Police Manual of
Guidance.
These forms are available as PDF documents. To view and print documents
