Claim forms
We have provided claim forms online for the benefits, allowances, pensions and other payments listed below.
Each link takes you to the information you need before you claim. Please read this information before filling in any forms. Each form also comes with notes that help you fill it in.
A choice of ways to claim
There are two types of Portable Document Format (PDF) forms available.
- In all cases we provide a PDF form that you can print out and fill in with a pen. These forms work in Adobe Reader versions 5.1 and above.
- In many cases we also provide a PDF form that you can save on your computer and fill in on screen. These interactive forms have a special feature that lets you save them so you do not have to fill in the form in one go. These forms work in Adobe Reader versions 7 and above.
How to use the PDF forms
Whichever type of the form you use, you must:
- print it
- sign it (or ask the claimant to sign it if you are not the claimant)
- send it by post to the address on the form or in the notes.
You can download the latest version of Adobe Reader free of charge from the Adobe website.
Do it online
In some cases you can provide and submit your details online instead of filling in a PDF form. We have used Do it online to indicate these cases.
