Please make sure you provide as much of the information asked for on the form as you can, as this will help us to decide whether or not we should carry out an enquiry.

Once you have sent the form to us by pressing the ‘Submit’ button you will receive an electronic message acknowledging receipt of your complaint. Please remember that we are unable, by law, to tell you whether or not we are investigating a particular company, or provide information which we may have obtained from our investigations.
