Licensing Act 2003 Application Forms

Application forms for the Licensing Act 2003 are now available. Applications for new personal licences, premises licences and club premises certificates, and applications to convert existing licences and certificates to the new permissions can be downloaded and printed below. Completed forms should be submitted to the relevant licensing authority.

Applications to convert existing licences and certificates must be submitted to the relevant licensing authority by August 6 2005. More information about the proposed timetable for implementation of the Licensing Act 2003 can be found at timetable for reform.

Please note that we hope to make forms that can be completed electronically available shortly.

Transitional provision forms  
For holders of existing licences or certificates wishing to convert (and vary) to new premise licences or club premises certificates during the transitional period.
