This month, the Adults facing Chronic Exclusion programme is highlighting the work of the ACE pilot in Merton, South West London.
Through a multi-agency partnership the New Directions Team (NDT) has been established and provides an early intervention for residents from Merton who are not engaging with frontline services, resulting in multiple exclusion, chaotic lifestyles and negative social outcomes for themselves, families and communities.
The team was created following the development of a local ‘chaos index’, to identify clients for the NDT. The NDT, through intensive case management and developing a collaborative working alliance have developed integrated pathways moving clients from at risk stages (e.g. repeat offending, discharge from NHS treatment, non-engagement in services, accommodation problems etc) to more structured engagement in services resulting in stable accommodation and employment or mainstream education/training.
We believe that the Index, and the collaborative approach used to devise it between key local agencies, is an idea that is highly replicable and highlights where there are specific gaps in services for chaotic people known to many, but worked with by few. The document below presents data on the Index and gives an overview of how it was developed for this project.
