Booking, Catering & Evaluation Form

  1. Complete one of the following booking forms:
    BIS Event booking form (for BIS staff only) Word document (155KB)
    Government and Agency Event booking form Word document (326KB)
    Non-Government Event booking form Word document (297KB)

  2. Send by email your completed form to futurefocus@berr.gsi.gov.uk
  3. Every event is tailored to your needs. You will be contacted by a facilitator to set up an initial meeting to plan the event, decide how long it should last, and set a date. They will also help you decide your objectives and clarify expectations on both sides. Prior to the event you will be asked to make a list of all participants and request any refreshments for the day plus advise any disability requirements or special needs.
  4. You should arrange your own catering to fit with the plan agreed with you by your facilitator. Easy online booking with Futurefocus preferred caterers can be found at www.bspoilt4choice.com
  5. The facilitator will then run the event.
  6. After the event you will be given a report containing all the data captured and asked to complete an evaluation form:
